• Must be a health care practitioner of some kind (pharmacy, doctor, chiropractor) or Indoor Environmental professional
• Must provide a valid EIN
• Must provide a practitioner or professional business license
• If reselling through a website, must provide URL

Program Requirements:

• Must support MAP pricing
• Cannot resell products on Amazon, eBay, or other online marketplace - can only sell through your own site or storefront
