Do we use "cookies"?
Yes. How can you opt-out, remove or modify information you have provided to us?
To modify your e-mail subscriptions, please let us know by modifying your preferences in the "My Account" section. Please note that due to email production schedules you may receive any emails already in production.
To delete all of your online account information from our database, sign into the "My Account" section of our site and remove your shipping addresses, billing addresses & payment information. Please note that we may maintain information about an individual sales transaction in order to service that transaction and for record keeping.

In an attempt to provide you with increased value, we may include third party links on our site. These linked sites have separate and independent privacy policies. We therefore have no responsibility or liability for the content and activities of these linked sites. Nonetheless, we seek to protect the integrity of our site and welcome any feedback about these linked sites (including if a specific link does not work).

When this notice applies:
This notice summarizes the privacy practices of Micro Balance Health Products, LLC and the Healthcare Professionals who utilize our Immune Response and Mold Sensitivity Evaluation (“IRAMSE”) to provide you with care or services. In addition, we may share information with each other for purposes described in this notice, including for our joint healthcare operations activities.
Our obligations:
We have a long-standing commitment to protecting the privacy rights of patients. In keeping with this commitment, and as required by law, we:
- Obtain your consent to use and disclose records about your health and healthcare through our Requisition form;
- Maintain the privacy of protected health information;
- Give you this notice of our legal duties and privacy practices regarding health information about you; and
- Follow the terms of our notice of privacy practices that are currently in effect.
How we may use and disclose health information:
When you utilize IRAMSE through your physician’s office, you will be asked to agree to an acknowledgment of this notice, which describes how we use and disclose information about you in ways that are permitted by federal law. The federal law provides for significant privacy protections of health information. All uses, disclosure of, or requests for protected health information (PHI) will be limited to the minimum amount necessary to accomplish the stated purpose. Professional judgment will determine the amount of information to be released. The minimum necessary standard is not intended to impede the provision of quality health care. Disclosure of PHI between providers for treatment and health care operations, or pursuant to an authorization without complying with this requirement are exempt from the minimum necessary rule.
The following categories describe ways that we may use and disclose health information that identifies you (“Health Information”). Except for the purposes described below, we will use and disclose Health Information only with additional written permission from you. If you give us permission to use or disclose Health Information for a purpose not discussed in this notice, you may revoke that permission at any time by sending a written request to our Privacy Officer at the address listed at the end of this notice.
a) For Treatment. We may disclose Health Information to your Physician (MD or DO), Naturopathic Doctor, Chiropractor, Nurse Practitioner, or Physician Assistant of other licensed professionals medical personnel (“Healthcare Professional”)
b) For Healthcare Operations. We may use and disclose Health Information for healthcare operations and administrative purposes. These uses and disclosures are necessary to make sure that patients receive quality care and for our operation and management purposes.
c) Research. Under certain circumstances, we may use blinded Health Information in aggregate for research purposes. That information would not contain unique personal identifiers that could reveal individual patients identities.
Special Circumstances
a) As Required by Law. We will disclose Health Information when required to do so by international, federal, state or local law.
b) Business Associates. We may disclose Health Information to our business associates who perform functions on our behalf or provide us with services if the information is necessary for such functions or services.
c) Health Oversight Activities. We may disclose Health Information to a health oversight agency for activities authorized by law. These oversight activities include, for example, audits, investigations, inspections and licensure as well as quarterly reports to the Agency for Healthcare Administration. These activities are necessary for the government to monitor the healthcare system, government programs and compliance with civil rights laws.
Your rights
You have the following rights regarding Health Information we maintain about you:
a) Right to Inspect and Copy. You have the right to inspect and copy Health Information that may be used to make decisions about your care. To request this information please contact us the address listed below and request the Patient Access Medical Record Request Form. Once the form is received and approved by the Privacy Officer, we will provide you with your Health Information.
b) Right to an Accounting of Disclosures. You have the right to request an accounting of certain disclosures of Health Information we made. Please contact us at the address listed below the Accounting of Disclosure Request Form. Once the form is received and approved by the Privacy Officer we will contact you with an answer to your request.

Complaints and questions
If you believe your privacy rights have been violated, you may file a complaint with us or the Secretary of the U.S. Department of Health and Human Services.
